If you are an established patient with Dr. Grant Miyashiro, please schedule your appointment with Dr. Miyashiro.
If you are an established patient with Dr. Lori Kim, please call our office at 935-8887. Dr. Kim will be unavailable while Eye Care Hawaii temporarily shares offices with Dr. Mike Miyashiro at 47 Lanihuli Street. Dr. Kim will return when our new building is finished at 34 W. Kawailani Street.
If you require an appointment on a day that your previously seen doctor is unavailable, please call our office so that staff can review your medical record prior to scheduling your appointment.

Patients we have not seen for three years or more should also download and complete the following forms. All patients seen within three years may simply update the current forms on file in our office.
